IT Band

The iliotibial band is a dense lateral thickening of thigh fascia that receives contributions from tensor fasciae latae and gluteus maximus and attaches distally near the lateral knee.
Definition
The iliotibial band (ITB) is a longitudinal thickening of fascia along the outside of the thigh. Its proximal anatomy has several layers and receives contributions from tensor fasciae latae and gluteus maximus; distally it forms a distinct layer at the lateral knee and attaches at Gerdy’s tubercle on the proximal tibia. It is connective tissue rather than a muscle and does not actively contract on its own.

IT band syndrome and HYROX®
Iliotibial band syndrome (ITBS) is one possible cause of activity-related lateral knee pain, especially in runners and cyclists. The older explanation of the band sliding back and forth over the lateral femoral epicondyle as a simple “friction syndrome” is disputed; compression of sensitive tissue beneath the band is a current alternative model, and sources describe several proposed mechanisms. HYROX® has 8 km of running, but no source reviewed here establishes ITBS prevalence among HYROX® athletes, a typical mid-race onset, or a protective ITB action at lunges and sled stations.

Training, rolling and symptoms
Evidence does not support diagnosing ITBS from a pace drop, soreness, hip weakness or running form alone. A 2023 review found that strength and kinematic findings differed by sex, current versus previous injury and study design. Stretching and release methods have not been studied enough to define a standard of care, so claims that the ITB can never change length, that rolling specific neighboring tissues provides relief, or that one strength routine prevents ITBS are too absolute. New, persistent or severe lateral knee pain needs assessment because several other conditions can cause similar symptoms.

Related terms
The ITB receives fascial contributions from tensor fasciae latae and gluteus maximus. Hip abductors and quadriceps have their own actions; being strong does not by itself prove reduced ITB loading.

Can I stretch or foam-roll my IT band?
The band is dense fascia, but current evidence does not justify an absolute claim that stretching or rolling can never affect the ITB complex. Their isolated clinical benefit remains unclear, and symptom care should be individualized.
