What testosterone is

Testosterone is an androgen hormone present in people of all sexes, although typical concentrations and clinical interpretation differ. It contributes to sexual function and to muscle and bone growth; in males it also supports sperm and red-blood-cell production. It is one part of a wider endocrine system, not a single controller of recovery, energy, glycogen storage or athletic performance.

What a result can and cannot show

A testosterone blood test cannot diagnose a health condition on its own. Symptoms, age, sex, medicines, health conditions, timing, assay and laboratory reference range all affect interpretation. For suspected male hypogonadism, the Endocrine Society requires compatible symptoms and signs plus unequivocally and consistently low concentrations, confirmed with a repeat morning fasting total-testosterone measurement. It recommends against routine screening of men in the general population. Evaluation in females follows different clinical questions and reference ranges.

Athletes, training load and low energy availability

Problematic low energy availability can affect endocrine function in female and male athletes. In the IOC REDs framework, clinically or subclinically low testosterone is an indicator for males, but every indicator requires consideration of other causes and the assessment tool must not be used alone for diagnosis. Fatigue, poor sleep, mood change or falling performance are non-specific. The ECSS/ACSM consensus likewise does not establish a single accepted hormonal test for overtraining syndrome; a testosterone-to-cortisol ratio cannot predict an impending collapse by itself.

Practical decisions

Resistance exercise can cause short-lived hormonal changes, but that does not establish that one lift, intensity or weekly frequency “optimises” long-term testosterone or performance. Adequate energy availability, recovery and an appropriate training programme support health, yet no reviewed guideline identifies a universal testosterone-boosting sleep target, fat percentage, zinc or vitamin-D protocol for HYROX® athletes. Persistent symptoms such as reduced libido, menstrual changes, unexplained loss of muscle or bone health concerns warrant assessment by a qualified clinician rather than self-diagnosis, annual screening or hormone/supplement treatment.

Frequently Asked Questions

Can heavy training lower testosterone?

Intensified training, inadequate recovery or problematic low energy availability can be associated with endocrine changes, especially in male athletes. A low result or non-specific symptoms still require clinical assessment and consideration of other causes.

Does a testosterone-to-cortisol ratio diagnose overtraining?

No. Overtraining syndrome is assessed from performance history, symptoms and exclusion of other causes. No single hormonal ratio is an accepted standalone diagnostic test.

Should every athlete test testosterone annually?

No universal annual-screening recommendation was found. Testing is most useful when a clinician identifies relevant symptoms, signs, prior abnormal results or another medical reason.
